April 6, 2026When I talk with patients about treatment-resistant depression, I want them to hear one thing first. You are not failing treatment. Treatment has not fully met your needs yet. That is an important difference. For some people, depression does not improve enough after trying standard treatment, or it improves for a while and then comes back. When that happens, it is time to look at other options with care, patience, and hope.
What Treatment-Resistant Depression Really Means
In simple terms, treatment-resistant depression usually means depression has not improved enough after trying at least two antidepressant treatments at the right dose and for the right length of time. That does not mean nothing can help. It means we need to think more broadly and more personally about the next step. That is where a thoughtful, whole-person plan matters.
Why EXOMIND® Is Such an Exciting Option
At our office, one of the newer options we are excited about is EXOMIND®. EXOMIND® is a noninvasive, drug-free treatment that uses magnetic stimulation to target parts of the brain involved in mood, focus, and self-control. It is FDA-cleared for the treatment of depression, and it is designed to fit into real life. Sessions are under 30 minutes, and people can return to their day after treatment.
What I like most about EXOMIND® is that it gives patients another path forward when traditional medication alone has not been enough. It builds on the science behind TMS, but it offers a newer platform that supports emotional regulation, cognitive function, and mental clarity in a way that feels practical and approachable. For the right patient, that can be a very meaningful part of care.
A Small but Important Mention: Spravato®
We also offer Spravato®, which is a prescription nasal spray for adults with treatment-resistant depression. For some patients, that different approach can matter, especially when other treatments have not brought enough relief. Spravato® is not the right fit for everyone, but it is another important option in the conversation.
Other Treatment Options Still Used Today
Even though our office is especially excited about EXOMIND® and also offers Spravato®, they are not the only modern options for treatment-resistant depression. Traditional TMS is still widely used today. Like EXOMIND®, it uses magnetic fields to stimulate nerve cells in the brain and is often considered when other depression treatments have not worked well enough.
Another option still used today is ECT, also called electroconvulsive therapy. ECT remains an important treatment for severe depression, especially when symptoms are intense or other treatments have not helped. There is also vagus nerve stimulation, or VNS, which can be used for some adults with long-lasting, hard-to-treat depression. These are all real, current options, and each one has its own place in care.
Why This Matters for Whole-Person Care
When depression lasts, it affects more than mood. It can affect energy, sleep, focus, motivation, confidence, and daily life. That is why I believe a whole-person approach matters so much. Medication and therapy are still important, but sometimes people need more than those two tools alone. Newer treatments can open the door to hope again, especially when care is built around the full person, not just a diagnosis.
